it'll be just a check over. see how she is growing. probably sound her chest, blood pressure etc. no more than that. injections, no, she needs parents consent for that. is she going with an illness?. are you an adult?. don't let the doctor do anything that your not happy or comfortable with.
just make sure they don't touch her in her no no spots
Chances are all they will do is check nose, ears, and throat, and heart rate, wieght and height. At 13 as long as she has been going to school she should have all the shots she needs. Tell her not to worry.

May be due for a tetnus shot, but otherwise don't know what else. Would require a parent or guardian to do that, so unless you are over 18 and have a letter authorizing shots, she probably won't get them.
If she is up to date with her shots, she doesn't have to worry. A physical is not bad, check your weight and height, push around on your belly, listen to your lungs, check your reflexes...It is a nothing. Piece of cake. It will be over in a flash. Also, worrying about it only makes it worse, do something with her today to take her mind off of it.

depends on her immunization history, if she is due to have one then she will get it. Physical is when doctor checks her from head to toe. Checking for vision via snellen chart, listening to heart, lungs and abdomen, neurological like reflexes and muscle strength. Request to be inside the exam room with her so she feels more relaxed. If she is not sexually active then the necessity of having a speculum exam is very minimal. Of every woman this age it is important to ask your doctor about the HPV (Human Papiloma Virus) vaccine that has recently come out. It is administered to patients her age and is extremely important for ridding this planet of cervical cancer. If she is due for any shots, she might get some.
First of all, if you're old enough to take her, how come you don't know?
If it's a complete physical that she's getting, they'll do a pap test where she has to get undressed and put on a gown. But first they'll take her blood pressure and stuff (oh, and if it's a male doctor, there will likely be a nurse in there as well, for liability issues, to make sure nothing unprofessional takes place). Then They'll insert something into her vagina to take a swab for testing (this doesn't really hurt, but she might feel something like a bit of a pinch, and it may bleed a bit too, so she should bring a pad just in case). If it is a complete physical they will also do a breast exam, and they may do blood tests as well, depending on if there's a specific reason that she's going, or what the doctor's opinion is.
If she's shy and worried about how embarrassed she'll be, tell her to just not think about it and it will be over before she knows it. The doctor and nurse who will be in there do this kind of thing all the time and won't even remember her once it's over, trust me.
